ok when in d does the car jerk when changing up or down?
is it better when the car is warmed up ie jerking disapears or is it all the time?

It only jerks when I've been sitting still at a set of traffic lights.
What happens
1/ I pull up at a set of lights
2/ I put it into to N whilst keeping my foot in the brake the whole time
3/ when I then want to pull away, I put it into D
This is when it starts to jerk.
I basically pulls the car hard, twice, like a hard rock forward.
It drives fine when it moves.
All gears work ok
But
When I then pull up at home, I put it in P.
Whilst the car is still running, I again press the brake n then I can't get it out of P

thats really odd my gearbox is on its way out and its fine when warm but jerks when cold maybe you need a fluid change

do a manual conversion if your going to break it you will be doing most of the required work anyway

sounds like you may meed to clean the filters in the shift control solenoid and change the fluid.
also adjust the selector cable.

here is a site i found when having trouble selecting reverse, your symptoms seem to be listed but im not sure you have the same autobox.
