Other than a couple of occasions, i dont recall that it actually ever occurred again. It was very rare, and was only ever when turning lightly.
I know we now have the 4ws manual now which might help? I havent read it so not sure what keeps the system aligned. The knowledge of that will probably point to what the system is trying to do.

That's exactly when mine makes the noise aswell.
I didn't know that we now had the 4ws manual, I'll check it out and keep you posted!

Could it be dirt in the gear pinion/rack?
Most likely unrelated, but I had a squeaking problem - whenever the rear wheels turned, I got horrible squealing from the motor (like an old bicycle, repetative high pitched "wee-wee-wee").
I removed the plug for the aligning pin, sprayed a fair amount of graphite oil / lube inside the motor and it went away.

Try cleaning out and lubricating through the locking pin hole like someone said above. It can't do any harm.

Regarding my noisy back end, I haven't found the problem itself but I did lift the ride height my about 5-10mm and now it's gone from doing it all the time to very rarely. I can only assume that having the car quiet low (I have no lips/skirts on the car) put extra strain on the steering arms and therefore the rack...not sure.
