Yeah your idle issue was an actual issue before... Your fan not kicking in isn't necessarily an issue. Have you been bleeding the coolant on level ground? Drive the car up a slight incline (front wheels higher) and then see if the coolant comes out the bleed valve. I had to make a header tank to bleed my 3rd gen properly recently as the radiator neck isn't really high enough to bleed the system with ease. Park it facing up a slight hill and try it then.
If the needle is at halfway then it's not overheating. That's where it should be.
Have you let the engine cool right down and then top the rad up to the bottom of the neck?

If the gauge hasn't gone above half way, then it hasn't got hot enough to worry about the fan not coming on, I'd say.
It does sound a bit odd that it hasn't come on if the engine had been running for 45 mins, but I guess it's not impossible?
What's the external temp up? Was the cabin heater running during this time?
If there was an air lock in the system, it is possible to fan could have been over-reactive before (i.e. coming on too soon) and you might just have cleared that now so all is back to how it should have been in the first place?
Like nafe says, try bleeding the nozzle and then topping up the rad with the car pointing uphill, just to be sure.

Don't let the fans not kicking in worry you. With the current outdoor temps, the car just being sat there isn't necessarily going to get hot enough within an hour. Only if you see the gauge go above halfway do you need to worry
