I think the TPS readings should be 0.5v closed and 4.5v open.

If the flap is sticking, then grease the shaft. Sound advice for all life problems

Ooo sticky flap now that would require further investigation...

Anyways no I don't think the TPS could cause 4K, if it was that far off it would just throw a code. If the plate isn't sticking I recon you've got a vac leak somewhere. Re-check all the vac lines at both ends with the engine off, then with the engine on give them a good firtle to see if it makes any difference. Next thing to check would be the TB gasket. Failing that it could be the IACV but I doubt it at as high as 4K.

Thought I had this sorted the tb sticking was the butterfly valve in the wrong way, it has a slight taper on one end and it's now opening smoothly and perfect.

But it's still bloody doin it. Yes we have blanked off the fitv, I thought it was common to remove these to stop it heating up the tb?

if you remove the fitv then you need aftermarket managment to compensate?

not sure exactly, but same thing happened to me, only after running the throttle body for a while

strange maybe its something to do with climate, or air temp?

tps gets confused?

There must be air getting in somewhere to rev all the way up to 4K. The IACV fully open would only take the revs to about 1.5-2K. I'd try going for a run with the intake pipe off completely so you have access to the TB, wait until the fault occurs and leave it revving high. Pull over and firstly check the throttle plate is completely closed by hand. Then stick your finger in the holes of the TB to indentify if it's FITV or IACV port related.
